Read the middle column again and a pattern appears. Almost none of these variables are visible in the moment. A trolley that crosses a zone line causes nothing today, and a candling error is invisible until the transfer count is reconciled. A training plan survives a busy week when it is short enough to be used during one. The plans we see abandoned are the ones written as a curriculum; the ones that hold are written as a progression tied to the production calendar, with named responsibility for each step.
The first ninety days, in three stages
- Observation with a purpose. The new operator shadows one station only, with a printed list of the four or five decisions taken at that station during a shift. The goal is not to help; it is to be able to name what is being decided and why.
- Supervised operation. The operator runs the station with the trainer present and free to intervene. Every intervention is written down, because that list becomes the real training content for the following weeks.
- Verified autonomy. The operator runs the station alone, and a supervisor observes a full task cycle at a moment the operator does not choose. Sign-off comes from that observation.
The third stage is the one most often skipped, and it is the only one that produces evidence. A signature collected at the end of a classroom morning tells you someone was in the room. An observation record tells you someone loaded a setter correctly under time pressure on a Thursday.
Procedures people will actually open
Standard operating procedures fail for a practical reason: they are written for the auditor and stored where the auditor looks. The versions that get used are one page, laminated, posted at the station they describe, and written in the order the task is performed rather than in the order the equipment manual describes it. We keep the long document for the quality file and derive the station card from it, never the reverse.
When a set point changes, the station card has to change the same day, otherwise the plant is running two procedures at once and only one of them is documented. This is also where the way work flows through the building and the way it is written down have to agree: a card describing a route nobody takes teaches people to ignore cards.
